SLPs receive 1.2 ASHA CEUs sponsored by the Stuttering Foundation. Please note that partial credit is not available for this convention. You must attend all three days to receive CEUs.

I give permission for my child(ren) to leave the hotel premises with adult Friends’ volunteers during the “outing” workshop.

I understand that the group will be walking within a few blocks of the hotel to practice stuttering “in the wild.”

By signing this form, I waive Friends, its officers, employees, volunteers, agents, and affiliates from any and all liability arising out of or related to any loss, damage or injury that may be sustained by my child while participating in the outing.
